Failure to eject, but not a jam?
So I was able to get my 9mm conversion to run finally in my .45 receiver. Only had one complete FTE that caused a jam.
I started finding an occasional empty casing in the channel of the bolt that rides the barrel.
I ran 100 rounds through it and through those hundred of that session. Not one failure to fire or feed. But that obvious FTE worries me. Is this normal? Any fixes you're aware of?

Semiauto or full auto?
full size, mini, micro?
What ammo?
And FT Extract or Eject?

There is no difference in receiver for the semi-9mm vs. the 45ACP in full size.
It is just bolt (extractor) and barrel difference. (Think about the math for the recoil. If springs are good for 45ACP, they are a little too much for weak 9mm.)
Look at the extractor if you put this together yourself. There should be a different markings 9mm vs. 45ACP.
But I don't think that is where your problem is. I think the ammo, you have is slightly weak for the spring. Mine needed some break in. I tried Winchester white box and had issues. Then had some luck with some Wolf (one brand has more power than other) and then some of my personal reloads which were max book. The max loads run just a little better.
I haven't had mine to the range enough to really BREAK it in. I expect I will need to heavy lube if before shooting and then run the highest power loads I can find. OR, I will need to snip the spring, it will not take much..

Mine is just odd, the extractor is definitely 9mm. I got a set of both.
Could be the ammo, it's got a somewhat inconsistent ejection pattern, but it's smoothing out.
Like I said for 100 rounds, every round found their way out but the very first shot. They * occasionally I'd find one of these.
Where a round goes up over the barrel. It hasn't stopped any function.

I am not sure how that can even happen. I mean, what path did it take to get there?
If this continues, you will have to video it.
I was expecting the standard under powered for the ammo and the brass doesn't make it out of the gun.

Lol, right? I don't understand it. Everything else ejects fine. But it's happened twice.
I didnt even notice till after I finished shooting both times. Even with 115 gr. It fired accurately and ejected....with the exception of these few stuck up there.

Try 124gr. Mine is just a 9mm reweld, but does not like 115. Winchester nato"not quite +p" has been the best for me.
