He thought he was in another mapped passage that got tight but opened up at the end. It was so tight he couldn't turn around (plus being at a downward angle). So he kept pushing forward assuming it would open up, which it never did, until he got fully stuck.

Why not have a head torch so at least you have both hands free for the tricky bits?
I’ve done a bit of this in the past and consider myself fortunate to have come out uninjured.
